LuxSci SecureForm comes full featured no matter if you have an existing LuxSci email account, a LuxSci Website, or host your website and/or email elsewhere.

Some features include:

• Security:
  • The process can be secure or insecure — We have you covered from a simple site that needs no encryption to complex sites requiring SSL encryption.
  • Takes care of many of the complex requirements that you face when setting up a secure web site. See Secure Web Pages and Web Forms: What You Need to Know.
  • Email Encryption: Use TLS Only, or PGP, or S/MIME encryption for secure email messages. Bring your own PGP or S/MIME certificate or have us generate one for you (if you have SecureLine or WebAides).
  • Enforced Security: Administrators can ensure that all SecureForms are configured with only secure settings.
  • Enable HIPAA-compliant web and PDF forms
• File Uploads: SecureForm supports the posting of uploaded files from your forms. Up to 25 files and 50 MB of data can be sent in each post.
• Data Collection Formats: Form data can be saved in the following formats
  • Plain text
  • CSV: Comma Separated Variables / Excel
  • Template File: A file with place holders that are replaced with your form data on submission. This file could be HTML, XML, or any other textual format.
  • Re-filled PDF: Upload a PDF form and have the submitted data re-filled into the form fields and saved either as an editable or not-editable PDF.
  • Submitted FDF: For PDF forms that submit their data as an "FDF file", these data files can be saved or sent to you.
  • Submitted PDF: For PDF forms that submit their data as an full "PDF file", these PDF files can be saved or sent to you.
• Emailed Form Data
  • Multile Recipients: Have the form submissions emailed to up to 10 different recipients.
  • Custom From/Subject: When capturing form data by email, you can customize the subject line, the 'From' name and address, and specify the recipient.
  • Dynamic Subjects: The Subject line of emailed data or notice messages can dynamically include the content of one or more of the posted form fields. (This feature is not available in HIPAA accounts, for compliance reasons).
  • Per-user encryption: Email encryption (if used) can use different methods for each recipients
• Post Notifications:
  • Receive emailed or text message/SMS notifications of each form post
  • Up to 10 recipients can receive notifications
  • Notifications can have custom from addresses and dynamic subjects
• Custom Success and Failure: For web site forms, you supply links to your own success and failure web pages; the end user is shown these once the form posting completes. For PDF forms, you supply your own success and failure PDF documents to be displayed to the end user once the form post is complete.
• Auditing: View and download detailed logs of successful form posts (IP address of user, time, secure or insecure?) and of failed posts.
• Internationalization: All form data is converted to UTF-8 for storage and delivery once it is received.
• Error Notifications: Notices of any problems with processing the form data are emailed to an email address of your choice.
• Anti-Spam: Block unwanted posts to your web forms via:
  • Optional Form Spam blocking without CAPTCHA (requires that the end user have JavaScript enabled and performs extended validation checks on the submitted data to block spam bots).
  • Optional HTTP referrer checking
  • Optional IP address allow and deny lists
  • Optional per-IP address rate limits; cut-off IP address(es) if limit exceeded over 24 hours.

• Branded URL for your SecureForm Posts, provided you also order an SSL Certificate and an IP for your own SecureForm domain name.

• Optionally have a form data files, and all files uploaded, saved to a Documents WebAide — LuxSci's file storage service.
• These WebAides are backed up daily and we provide free restores.
• The data saved to these WebAides can be accessed securely any time from any Web Browser.

With a MySQL Database and Web Hosting Services:

• You can have your form posts saved automatically to your MySQL database
• MySQL database provides automated secure remote access to form submissions

• Changes to your form fields are automatcially reflected in the MySQL database, with no work required by you.

With a Dedicated SecureForm Server

• No explicit limits on the number of form posts per day and the amount of form data processed per day.

Your Perspective: The Form Administrator

SecureForm is simple and turn-key:

• You design your own web site forms and/or PDF forms
• You use our web interface to configure what should happen to your form data (i.e. post the data securely or not, email it or archive it, where to send people on success, etc.)
• You point your form to a web address that we provide to you
• .... That's it.

The End-User Perspective: Web Site Forms

• The user goes to your form on your web site, wherever it is hosted
• The user fills out your form
• The user presses your form's "Submit" button.
• The data is sent [securely] to LuxSci at a web site address that we provided to you.
• On success, the user is redirected to your web site page for successful results.
• On failure, the user is redirected to your web site page for failure results.

Unless your form is misconfigured, the end user will always be on your web site and always see your URLs in his/her address bar. With web site forms, the submission process to LuxSci'servers is always fully transparent to the user.

The End-User Perspective: PDF Forms

• The user opens your PDF form
• The user fills out you form
• The user presses your form's "Submit" button.
• The user may be prompted to confirm that they trust the address to which the form data is going. (This will be the address provided to you by the LuxSci system; it can be your own domain through Private Labeled branding with the purchase of a dedicated server)
• The data is sent [securely] to LuxSci at that web site address.
• On success, the user is shown your custom "Success PDF".
• On failure, the user is shown you custom "Failure PDF".

Quick and easy - They press a button in the PDF, confirm the submission, and they are done.

SecureForm Tiered Pricing:

Our tiered pricing is based on the number of form posts or amount of inbound data* posted per day across all forms in your account. SecureForm is affordable and competitive to make it easy for you to process form data and only pay for what you use.

The price list below includes the prices for SecureForm as a "standalone" service (which also includes an individual @luxsci.net address, 2GB of storage, email access, and a WebAide license), and as an upgrade to any other kind of LuxSci account.

Daily Usage Limit Price

*The amount of inbound data posted includes the size of all uploaded files as well as the amount of data used to upload the form fields and their respective filled in values. If you have your form data emailed to two or more recipients, then the effective amount of posted form data is taken as the actual amount of posted form data times the number of email recipients.

**For large accounts, dedicated servers are recommended. These start at about $300/mo for SecureForm and do not have explicit limits on posts of amount of data posted.

Soft Limits

If your account exceeds its SecureForm daily limit, then it automatically starts using the "next tier" of service. At the end of the month, your account will be billed 150% of the difference in price for your current tier of service and the next tier to cover this supplemental usage.

• If you upgrade before the end of the month, there will not be any supplemental overage fee.
• If you change your service limit type from "soft" to "hard", then your forms will be cutoff until midnight GMT if your account hits its limit, rather than using supplemental limits
• If your account is using supplemental limits and hits the maximum daily usage for the next tier as well, then form usage will be suspended until midnight GMT.

For example, if your account has a 100 posts/day limit:

• If you change your limit to "hard", then after 100 posts, no other posts will be accepted for any of your forms until midnight, GMT, when your daily limits reset.
• With the default soft quota, your usage can spike up to the next tier of 500 posts/day without being cut off at all
• With the default soft quota, if your usage exceeds 100 posts/day during any day of the month, you will be charged for supplemental usage at the end of the month. In this case, that would cost: ($35.00 - $20.00) x 150% = $22.50. Of course, if you upgrade to the 500 post/day tier there is no supplemental usage fee.
